2/12/2023 EnchantaVaughn Approved 1. Behavior: When QMHP came to check on the client at her residence, the client was witnessed sitting in the living room playing with her little sister. The client was present to her environment and was able to enjoy her time and space in the home. Client appeared to be in a pleasant mood, as evidenced by her eagerness to clean up and join her counselor in a one on one session. The client was calm and engaged in full conversation with counselor about her day and the current emotions she was feeling. 2. Behavior: Client communicated she was comfortable with the selected activities, implementing excellent manners and showing respect to everyone. Client was witnessed assisting her mom with duties around the house and sharing her favorite things with her sibling. Client was applauded for her sportsmanship skills and presenting excellent behaviors. Client responded with positive gestures and continued following along with each prompt given by QMHP. 1. Intervention: QMHP provided client with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP praised client for staying on task and working well with her family. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of today’s session. QMHP modeled pro-social behaviors for client to strengthen her understanding of what to do in school, at home, and in the community. 2. Intervention: QMHP introduced client to an activity related to her mental health objectives. The activity’s objective is to increase self-esteem by receiving positive comments from others. The activity was also completed to receive positive comments about themselves. Each family member had to give positive comments about each other. Each family answered the discussion questions fairly and seemed to be learning a lot about each other. QMHP gave a brief summary of the responses and the overall lesson from the exercise. 1. Response: Client was attentive and engaging in conversation throughout the session. Client verbalized positive feelings about herself and others. Client showed good understanding of what it means to be respectful, encouraging, mindful, and fair to others. Client was in a pleasant mood. Client was seen having a good evening. Client was able to identify the role she plays in each situation and continues to comply with authority rules and regulations. 2. During the session at the home, mother expressed client’s behaviors have improved but she still has to remind client about remaining consistent with reacting positively when being reprimanded or redirected. Mother and client displayed good communication. Although mom is more stressed with the end of her pregnancy, she has cut back on work to spend more time with her daughter. Client responds well to praise and encouragemeResponse: Client discussed mental illness and its causes as a family. Client displayed interest in learning about ADHD. The family shared the things that they noticed changed in client’s behavior patterns, which assisted in creating a more positive environment and a healthier family relationship. Client demonstrated expectations and responded well to feedback. Client reported a decreased stress level due to her modulation of social contact, and the success of this was reviewed.
